Kindergarten & 1st Grade Registration Information
Kindergarten and First
Grade Registration for the 2010-2011 school year will take place on Wednesday, March 24, 2010, from 9:00 am to 12 noon, at Valentine Elementary School Little Theatre. An appointment is not necessary to register on
March 24. Registration for all other grades and those who are
unable to register on March 24 will take place on Wednesday, April 28, 2010, 8:00 am to 12:00 noon, in the District Office Boardroom, and again on Wednesday, June 30, 2010, 1:00 pm to 5:00 pm, in the District Office Boardroom.

STEP 2: Print, Complete and Bring the Following Forms.

[B] Kindergarten Placement Information Sheet
[C] TB Test Requirements & Test Report
[D] Health Requirements Information and Forms [E] Oral Health Assessment / Waiver Request Form
STEP 3: Gather and Bring the Following Documents (Originals and copies). (Chinese Version)

CURRENT PROPERTY TAX BILL OR CURRENT LEASE AGREEMENT OR CLOSING ESCROW STATEMENT;
AND
4 CURRENT UTILITY BILLS (electric, gas, water, trash, cable, telephone [not cell phone])
are required for proof of residency in the San Marino Unified School District.

BIRTH CERTIFICATE AND/OR PASSPORT
for certification of birth date.

SOCIAL SECURITY NUMBER
if applicable.

COMPLETE, UP-TO-DATE, IMMUNIZATION RECORD
signed by a physician. All the immunizations required by the State of
California are listed in the registration packet. Our district strictly
enforces the “NO SHOT - NO SCHOOL” policy.

MANTOUX TUBERCULOSIS SKIN TEST
for all students entering school in California for the FIRST time. This
test must be given and verified in writing by a physician. The form is
supplied for this requirement.

REPORT OF HEALTH OF EXAMINATION FOR SCHOOL ENTRY,
for Kindergarten and First Grade only, is required by the State of
California and must be completed and signed by a physician.
