6th thru 12th Grade Registration Information

Registration is processed by appointment only.  All registration materials must be completed before making an appointment to register.  The free Adobe Reader is required to view the PDF files.

STEP 1:  Important Information You Must Read First.

STEP 2:  Print, Complete and Bring the Following Forms.

[A]  School Registration Form
[B]  TB Test Requirements & Test Report
[C]  Food Services Payment Form
[D]  Application for Free and Reduced-Price Meals or Free Milk (Spanish)

STEP 3:  Gather and Bring the Following Documents. (Chinese Version)

  1. CURRENT PROPERTY TAX BILL OR CURRENT LEASE AGREEMENT OR CLOSING ESCROW STATEMENT;
    AND

    4 CURRENT UTILITY BILLS

    are required for proof of residency in the San Marino Unified School District.
     

  1. BIRTH CERTIFICATE AND/OR PASSPORT
    for certification of birth date.
     

  1. SOCIAL SECURITY NUMBER
    if applicable.
     

  1. COMPLETE, UP-TO-DATE, IMMUNIZATION RECORD
    signed by a physician. All the immunizations required by the State of California are listed in the registration packet. Our district strictly enforces the “NO SHOT - NO SCHOOL” policy.
     

  1. MANTOUX TUBERCULOSIS SKIN TEST
    for all students entering school in California for the FIRST time. This test must be given and verified in writing by a physician. The form is supplied for this requirement.
     

  2. OFFICIAL TRANSCRIPT
    from the Student's previous school (translated into English if necessary).
