6th thru 12th Grade Registration Information
Registration for the 2010-2011 school year for all grades will take place on Wednesday, April 28, 2010, from 8:00 am to 12:00 noon in the District Office Boardroom, and again on Wednesday, June 30, 2010, from 1:00 pm to 5:00 pm in the District Office Boardroom.All registration materials must be completed to register. The free Adobe Reader is required to view the PDF files.

STEP 2: Print, Complete and Bring the Following Forms.
[A] School Registration Form
[B] TB Test Requirements & Test Report
[C] Food Services Payment Form (optional)
[D] Application for Free and Reduced-Price Meals or Free Milk (Spanish) (optional)
STEP 3: Gather and Bring the Following Documents. (Chinese Version)

CURRENT PROPERTY TAX BILL OR CURRENT LEASE AGREEMENT OR CLOSING ESCROW STATEMENT;
AND
4 CURRENT UTILITY BILLS (electric, gas, water, trash, cable, telephone [not cell phone])
are required for proof of residency in the San Marino Unified School District.

BIRTH CERTIFICATE AND/OR PASSPORT
for certification of birth date.

SOCIAL SECURITY NUMBER
if applicable.

COMPLETE, UP-TO-DATE, IMMUNIZATION RECORD
signed by a physician. All the immunizations required by the State of
California are listed in the registration packet. Our district strictly
enforces the “NO SHOT - NO SCHOOL” policy.

MANTOUX TUBERCULOSIS SKIN TEST
for all students entering school in California for the FIRST time. This
test must be given and verified in writing by a physician. The form is
supplied for this requirement.

OFFICIAL TRANSCRIPT
from the Student's previous school (translated into English if necessary).
